Fees are due at time of service and are subject to change.

Intake forms are now electronic and can be sent to you through your email. This will allow you access to our client portal where you can schedule appointments, update demographics, add insurance information, and pay your bill.

Therapy Session, 45/50 minutes: $150

Therapy Session, Interactive (done in ASL), 45-50 minutes: $175

Life-Coaching/Consulting

• Option 1: Single 1.5 hour session, $200

• Option 2: 4 pre-paid sessions, $500

Presentations Offered to Church Groups: No Charge

Seminars and Classes: Nominal Fee, Dependant on Group Size

Late Cancellation/No Show Fee: Unless 24 hours notice is given, the full amount of the session will be charged to the client.

Please give 24 hour notification, for appointment cancellation, to avoid the late cancel fee.

Currently accepting new clients from the following insurance companies: Aetna, Altius (MHNet), Blue Cross Blue Shield, Deseret Mutual/DMBA, Educator's Mutual/EMI Health, Health Utah, Select Health, Smith Administrators, Tall Tree Administration, and Utah Behavioral Health.

Please call your insurance company to see if LSC is an on-panel provider, or if you have out-of-network coverage, and if a pre-authorization is needed. We can not guarantee coverage.

Regretfully, we do not currently accept: Medicaid, Medicare, PEHP, WISE Provider Networks/Summit Care.

You're welcome to pay out of pocket and submit for reimbursement from your insurance provider.
